About Our Agency
Our agency has a genuine desire to provide a caring support system for individual members of our community in need. Going above and beyond is written into our policies and we pride ourselves on accepting a wide range of individuals, many of whom have struggled to find assistance in the past.
Our agency strives to keep members of the community in their homes for as long as they would like, rather than being required to enter a hospital or nursing facility long term. Additionally, we provide short term assistance in the home following acute illness, injury, or diagnosis exacerbation. We accomplish these objectives by offering an array of nursing and therapy services provided by highly trained, trusted, experienced, and credentialed professionals.

We work directly with each patient’s provider. Communication is constant throughout the course of treatment.

Care from Your Community
Although the corporate office is located in Delta, Ohio, our staff members themselves reside throughout the Northwest, Ohio region ensuring proximity to the patients they serve. A Registered Nurse who lives closest to the patient is assigned to manage the patient-specific home care needs, working as a liaison between the physician, the therapists, and other providers to ensure continuity of care.

Using the expertise of nursing and therapy, the goal of Dedicated Nursing & Rehabilitation at Home is to provide compassionate care to each patient we serve ensuring peace of mind for themselves and their family. We aim to assist with the patient’s return to as full and rich a life as possible after suffering a debilitation injury or illness, and to provide community support assistance for as long as is requested or necessary to those in the community that need it.
