Upon admission to our Agency, a Registered Nurse under the supervision of the patient’s physician, assesses the patient and develops a customized plan of home health care services addressing the patient’s individual home health care needs.

 

Although the corporate office is located in Delta, Ohio, our staff members themselves reside throughout the Northwest, Ohio region ensuring proximity to the patients they serve. With this in mind, a Registered Nurse who lives closest to the patient is assigned to manage the patient-specific home care needs, working as a liaison between the physician, the therapists and other providers to ensure continuity of care.

Dedicated Nursing & Rehabilitation at Home strives for patient satisfaction with our home care services. We recognize the need for home care services to be conducted in a “patient-friendly” environment, but we also recognize that unfortunately personality conflicts can exist between the patient and one of our staff members. Should such a conflict exist, we are happy to provide an alternate home health staff member until we get the right mix.
